Information You Need To Know

  • Pictures will be taken the day of registration (Make-up Day Sept. 7th). You may order pictures online at (school code AA0809). If you choose to order online please bring in your receipt with you for pictures.
  • Free and Reduced Lunch Information 2018-2019.
  • Medication Form—All medications (prescription and over the counter) MUST have parent and physician approval/documentation (new this year).
  • Yearbook—Yearbook can be ordered through
  • HMS PAC—HMS Parent Advisory Council (PAC)—Volunteer—Coming Soon.
  • Spirit Wear—Coming Soon. 
  • Sports Physical—Must be dated after April 15, 2018. Must have a sports physical to tryout and/or participate in any sport. Form MUST be completed by a physician, parent, and student.
    • Volleyball tryouts will be starting on August 29th right after school in the gym.
    • Cross Country starts August 28th (first day of school). Check out the Cross Country page for more information.
  • 8th Grade OnlyApplication for Financial Assistance for 8th Grade Trip(s).
  • Affidavit of Fitness for School District Volunteers—Fill this form out if you plan on volunteering as a chaperone or classroom volunteer
School Supplies
Teachers will post suggested school supply lists outside classrooms during registrations days. We will also have free recycled school supplies available.
